| NRC BULLETIN 91-01 RESPONSE, 4-HOUR NOTIFICATION - LOSS OF MODERATION AND |
| MASS CONTROL |
| |
| On 12/09/98 at 1820 EST, Plant Operations and Nuclear Criticality Safety |
| personnel identified that a Nuclear Criticality Safety Analysis (NCSA) |
| requirement could not be maintained in the X-326 building. NCSA-326_13.A06 |
| administrative control requirement #8 states, in part, "Within 8 hours after |
| cascade equipment shutdown (motor turned off) and with the system at a UF6 |
| negative, the system shall be pressurized with dry air or N2 buffer at 14 |
| psia or greater." This cannot be accomplished for cell 25-7-2 due to the |
| primary containment that was breached during a fire that occurred in the |
| cell. Also, cell 25-7-6 leaked to atmosphere prior to being buffered due to |
| possible failure of an expansion joint. Each cell contains 12 converters, |
| and the holes in the converters caused a loss of moderation control and loss |
| of mass control. The holes in the converters can let air intrude into the |
| converters which can cause a buildup of moisture in the converters. |
| |
| The certificate holder has isolated both cells and isolated the water system |
| going to both cells including the sprinkler system (fire protection system). |
| Visual inspection of a limited number of the converters and a review of |
| typical cell inventory data reveals that the cells do not contain a critical |
| mass in them. Also, the cells mentioned above have been isolated. |

| The certificate holder notified the NRC Resident Inspector, the local |
| Department of Energy (DOE) representative, and the state of Ohio. The |
| certificate holder also issued a press release. |

| This event notification is being updated to change the previous report of a |
| 4-hour notification (loss of moderation and mass control) to a 24-hour |
| notification due to current NDA scans that identify the deposits in |
| converter 25-7-2 to be less than the calculated safe mass. Loss of a single |
| contingency (moderation) remains a 24-hour notification. Also, it was |
| determined that the previous report of converter 25-7-6 leaking to |
| atmosphere (potential loss of moderation control) was incorrect. There was |
| no failure in converter 25-7-6 that breached containment. |

| The certificate holder notified the NRC Resident Inspector, the local DOE |
| representative, and the state of Ohio. The NRC Operations Officer notified |
| the R3DO (Mark Ring), NMSS EO (Wayne Hodges), and AEOD IRD (Joe Giitter). |

| UPON FURTHER REVIEW OF THIS EVENT, IT HAS BEEN DETERMINED THAT THE EVENT |
| MEETS THE REPORTING CRITERIA FOR OPERATIONS THAT COMPLY WITH THE DOUBLE |
| CONTINGENCY PRINCIPLE WHERE MODERATION IS USED AS THE PRIMARY CRITICALITY |
| CONTROL. THE REPORTING CRITERIA CHANGED WHEN THE DEPOSIT IN 25-7-2 WAS |
| DETERMINED TO BE LESS THAN A SAFE MASS. NCSA-326_013.A06 ADMINISTRATIVE |
| CONTROL REQUIREMENT NO. 8 WAS NOT RE-ESTABLISHED WITHIN 4 HOURS AFTER THE |
| INITIAL OBSERVATION OF THE EVENT. THAT CONTROL IS MODERATION, AND THE |
| MODERATION CONTROL HAS YET TO BE RE-ESTABLISHED. MASS CONTROL WAS |
| MAINTAINED THROUGH THE EVENT. THIS MEETS THE BULLETIN 91-01 4-HOUR |
| REPORTING CRITERIA. THIS EVENT SHOULD NOT HAVE BEEN DOWN GRADED TO A |
| 24-HOUR REPORT. |

| Upon further review of this event, it has been determined that the event |
| constituted only a single contingency in that the operation of the cascade |
| is considered a single contingency in accordance with SAR 5.2. Thus, the |
| report of a double contingency loss was incorrect and the only contingency |
| lost was moderation. The report would still be an initial 4 hour |
| notification requirement. |

| Portsmouth personnel notified the NRC Resident Inspector and the DOE site |
| representative. The NRC Operations Officer notified the R3DO (Melvyn |
| Leach), NMSS EO (Robert Pierson), and IRO (Joseph Giitter). |

| INADVERTENT ACTUATION OF SINGLE OFFSITE SIREN DURING TESTING OF ACTUATION |

| |
| During Y2K testing of the offsite siren actuation system, a single siren |
| actuated for about 5 seconds. The test was terminated and the system |
| returned to pretest conditions. The licensee notified the Louisiana Office |
| of Emergency Preparedness (LOEP) and informed them of the actuation. The |
| LOEP decided not to make a public announcement on the emergency alert system |
| as the siren was only actuated for a short period of time. |

| The licensee notified the local parishes and the NRC Resident Inspector. |

| High Pressure Core Spray (HPCS) Division 3 Diesel Generator declared |
| inoperable. |
| |
| |
| HPCS (Division 3) EDG oil level was discovered to be out of specification |
| low during normal rounds. The Division 3 EDG was conservatively declared |
| inoperable until further evaluations can be performed to determine final |
| disposition of oil leakage from its inspection cover. The licensee entered |
| Technical Specification 3.8.1 (72 hour limiting conditioning of operation). |
| All Emergency Core Cooling systems including HPCS are fully operable. |
| Earlier in the day an undervoltage spike on the electrical grid caused the |
| Division 3 EDG to start. The diesel was properly secured. |

| The NRC Resident Inspector was notified of this event by the licensee. |

| TSR allowed one of two smoke detectors to be inoperable during withdrawal |
| operations when both smoke detectors have to be operable during withdrawal |
| operations. |
| |
| On March 10, 1999, Operations personnel identified a deficiency in a TSR |
| requirement for the Pigtail line Isolation System at ERP (extended range |
| product), LAW (low assay withdrawal), and TAILS, (TSR 2.5.3.4). The TSR |
| required action allowed continued withdrawal operations if one (1) smoke |
| detector was inoperable. The Pigtail Line Isolation System requires two (2) |
| Pyrotronics Smoke Detectors to actuate for the isolation valve to close. |
| Operation personnel initiated a Compensatory Action (CA-99-009) for this |
| anomaly to isolate the withdrawal cylinder and manifold per TSR 2.5.3.4b if |
| one smoke detector is declared inoperable. |
| |
| Initially there was no evidence that the Pigtail Line Isolation System had |
| been allowed to operate any applicable mode (modes II or III) with one or |
| more inoperable smoke head. As a follow up investigation to this |
| deficiency, a search of the BPS system (problem report database) was |
| conducted. This search identified that on July 8, 1998, a Pyrotronic smoke |
| head at the Law IA withdrawal position had been declared inoperable and |
| cylinder filling operations had been allowed to continue. |
| |
| This event is reportable due to a Safety System required by the TSR not |
| being available or operable and no redundant equipment was available to |
| perform the required safety function. |
| |
| The NRC Resident Inspector was notified of this event by the certificate |
| holder. |

| LOCAL LEAK RATE TEST PERFORMED ON THE "A" LOOP FEEDWATER CONTAINMENT |
| PENETRATION FAILED. |

| An Appendix "J" Local Leak Rate Test was performed on the "A" Loop |
| Feedwater containment penetration. The penetration piping was unable to be |
| pressurized and therefore the leakrate exceeded Tech Spec limits. The |
| penetration is isolated and the condition does not represent an immediate |
| threat to the public health or safety. |

| VALID ACTUATION OF A 'Q' SAFETY SYSTEM |
| |
| "At 1003 hours on March 25, 1999, during normal charging of cell 31-3-3 |
| prior to placing onstream, an Operations |
| Front Line Manager was investigating a Cascade Automatic Data Processing |
| smokehead alarm from this cell when a second smokehead alarmed. He than saw |
| what appeared to be smoke above the Stage 6 area. |
| |
| "All personnel in the area immediately evacuated, implemented the 'See & |
| Flee' Procedure, activated the building recall system, and notified |
| Emergency Response Forces. The cell was taken offstream and cell pressure |
| was reduced below atmosphere. |
| |
| "All airborne and surface contamination sample results were less than |
| detectable and there was no release of |
| Hazardous/Radioactive material or Radioactive/Radiological contamination |
| exposure as a result of this event." |

| The facility notified the NRC Resident Inspector and the DOE Site Manager. |

| Incident report 7431: A Texas licensee - Team Consultants, reported that a |
| Troxler 3411B, SN 11125, Moisture Density Gauge containing Cs-137 and |
| Am-241:Be was stolen. The Texas Department of Health Bureau of Radiation |
| Control received the notification letter on 3/22/99 and is following up to |
| obtain additional information. |

| ESF ACTUATION RESULTING IN ISOLATION OF A DIVISION 2 VALVE |
| |
| "An invalid ESF actuation resulted in the isolation of Division 2 primary |
| containment isolation valve RHSAOV62 SUPPRESSION POOL CLEANUP (SPC) SYSTEM |
| SUCTION. |
| |
| "During outage preparation activities in the Reactor Building Dryer storage |
| pool, level in the pool was inadvertently lowered approximately three and |
| one-half inches, resulting in a non-ESF isolation signal from the division 2 |
| dryer pool level indicator. The level change was caused by a siphon effect |
| that was created through a vacuum hose being staged between the Dryer Pool |
| and Refueling Cavity for pool cleanup. |
| |
| "The lowering level was terminated upon identification and all systems |
| responded as designed." |
| |
| The siphon action was started during 'bump' testing of the vacuuming system |
| during setup due to the differences in pool levels of about one foot. Work |
| has been terminated until an investigation is completed and necessary |
| changes to the work are made. |
| |
| The licensee notified the NRC Resident Inspector. |
