| BULLETIN 91-01 4-HOUR REPORT                                                 |
|                                                                              |
| The following text is a portion of a facsimile received from Portsmouth:     |
|                                                                              |
| "On January 8, 1999, at 1230 hours, it was discovered that no Nuclear        |
| Criticality Safety Approval (NCSA) was in place for the X-326 Evacuation     |
| Booster Station.  The Evacuation Booster Station has been abandoned in place |
| for more than 20 years.  A UF6 negative was obtained on this equipment       |
| before it was isolated from the rest of the X-326 cascade equipment.         |
| Applied Nuclear Technology (ANTS) is analyzing to determine if greater than  |
| 15 grams of U235 is present."                                                |

| "This event is being reported under criticality control (Bulletin 91-01)     |
| 4-hour due to the potential that abandoned equipment contains more than 15   |
| grams U235 requiring NCSA per [Technical Safety Requirement] 3.11.2."        |

| The NRC resident inspector was notified by Portsmouth personnel.             |

| The following text is a portion of a facsimile received from Portsmouth:     |
|                                                                              |
| "[Nondestructive analysis (NDA)] results obtained from the X-326 [Evacuation |
| Booster Station] indicated numerous components containing greater than 15    |
| grams U235 present.  Enrichment was a maximum of 26% U235 with the mass      |
| deposits determined to be less than safe mass.  Two components were          |
| determined to have a U235 mass requiring the components to be handled as     |
| 'PEH' [(Planned Expeditious Handling)]."                                     |
|                                                                              |
| The NRC resident inspector has been informed of this update by Portsmouth    |
| personnel.  The NRC Operations Officer notified the R3DO (Gavula), NMSS EO   |
| (Surmeier), and IRD (Congel).                                                |

| PLANNED SHUTDOWN DUE TO INDICATIONS OF DEGRADATION ON THE DIVISION I DIESEL  |
| GENERATOR OUTPUT BREAKER CURRENT TRANSFORMERS                                |
|                                                                              |
| The following text is a portion of a facsimile received from the licensee:   |
|                                                                              |
| "[During a planned maintenance outage, it was discovered that the] Division  |
| I diesel generator output breaker current transformers (CT) have indications |
| of degradation.  [A small puddle of CT encapsulant material was noticed on   |
| the cubicle floor when the breaker cubicle was opened.]  This degradation is |
| suspected to be related to a [10 CFR] Part 21 report  from ABB Power         |
| Distribution, Inc., on CT encapsulant material dated April 17, 1989.  The    |
| [Division] I [diesel generator] is currently inoperable.  For personnel      |
| protection, it is necessary to shut down the unit to complete inspection and |
| repair.  The unit is shutting down prior to the [technical specification]    |
| time limit, and [the unit] is not currently in the shutdown action           |
| statement."                                                                  |
|                                                                              |
| At the time of this notification, the unit was approximately 24 - 25 hours   |
| into a 72-hour limiting condition for operation for planned diesel generator |
| maintenance activities.  The unit shutdown is tentatively planned to         |
| commence at 0030 on 01/13/99.  The licensee expects that the unit will enter |
| Mode 3 (Hot Shutdown) some time on day shift, and the licensee plans to      |
| place the unit in Mode 4 (Cold Shutdown).  The current estimated restart     |
| date is 01/19/99.                                                            |
|                                                                              |
| At this time, the licensee does not believe that this issue affects the      |
| other diesel generator because surveillance tests were successfully          |
| completed on it last week.  However, the impact on the other diesel          |
| generator is still being evaluated.                                          |

| The licensee notified the NRC resident inspector.                            |

| LOSS OF HALF OF THE OFFSITE EMERGENCY SIRENS DUE TO FREEZING RAIN            |
|                                                                              |
| The following text is a portion of a facsimile received from the licensee:   |
|                                                                              |
| "10CFR50.72(b)(1)(v) requires that a facility notify the NRC Operations      |
| Center during any event that results in a major loss of emergency offsite    |
| response capability.  Due to weather conditions causing area power outages,  |
| a system test of the emergency sirens was conducted.  During this test, it   |
| was discovered that 22 out of 44 emergency sirens were not functioning.      |
| Therefore, Clinton Power Station is making a notification pursuant to        |
| 10CFR50.72(b)(1)(v)."  The licensee's current site policy is to make a       |
| notification of this type if less than 90% of the emergency sirens are       |
| available.                                                                   |

| FIRE DAMAGED CONTAINER OF DRY PRESS CAKE.                                    |
|                                                                              |
| An operator noticed a small fire in a polypack located in a ventilated hood  |
| which services an oven used to dry press cake.  Apparently, the press cake   |
| residue had not cooled sufficiently, and it ignited inside the polypack.     |
| The fire was promptly extinguished by the operator.  Damage was limited to   |
| the polypack, which was partially melted during the brief fire.              |
|                                                                              |
| There were no significant personnel exposures as a result of this event, no  |
| significant elevated air sample results, and no releases to the              |
| environment.                                                                 |
|                                                                              |
| The Incident Review Committee convened January 13, 1999, to review the event |
| and determine that appropriate actions are being taken to address the        |
| event.                                                                       |
|                                                                              |
| The licensee notified the NRC Region II office (David Ayres).                |

| VARIAN NICKEL-63 GAS CHROMATOGRAPH DETECTOR MISSING FROM CHICAGO DISTRICT    |

| There were five detectors at the Food and Drug Administration's Chicago      |
| District Office when the laboratory was closed approximately 1 year ago.  At |
| that time, three detectors were shipped to Winchester, Massachusetts; one    |
| was shipped to Arkansas; and one has been missing for approximately 1 year.  |
|                                                                              |
| The missing detector was a model-600 GC-type detector (model #02-001972-00,  |
| serial #A2980) which contained 10 - 15 mCi of Nickel-63.  It was used to     |
| detect pesticides in foods.                                                  |
|                                                                              |

| OFFSITE NOTIFICATION REGARDING OIL SHEEN IN COLUMBIA RIVER                   |
|                                                                              |
| The licensee notified the Coast Guard and the Oregon Emergency Management    |
| Division regarding the sighting of an oil sheen on the Columbia River.  The  |
| licensee reported that, at 0800 PST on 01/12/99, a small quantity of oil     |
| (less than one pint) leaked out of a drop box located on the intake          |
| structure access road and was washed into the river by rain.                 |

| 24-HOUR REPORT IN ACCORDANCE WITH OPERATING LICENSE DUE TO INADEQUATE        |
| TESTING OF STATION BATTERIES                                                 |
|                                                                              |
| The licensee reported that  testing of the 'B' and 'D' station batteries was |
| not performed in accordance with Technical Specification (TS) 4.8.2.1e due   |
| to improper application of a test waiver provided in TS 4.8.2.1d.            |
|                                                                              |
| The following report was submitted by the licensee via facsimile:            |
|                                                                              |
| "Technical Specification 4.8.2.1d requires a service test every 18 months.   |
| A service test is a discharge test that demonstrates the ability of the      |
| battery to meet the design load profile of the DC system under accident      |
| conditions.  Technical Specification 4.8.2.1e requires a performance test    |
| every 60 months.  A performance test is a discharge test that measures the   |
| capacity of the battery against the manufacturer's rated capacity.  This     |
| Specification also states, 'Once per 60-month interval this performance      |
| discharge test may be performed in lieu of the battery service test required |
| by Specification 4.8.2.1d.'  Technical Specification 4.8.2.1f requires a     |
| performance test every 18 months when a battery reaches 85% of its design    |
| life."                                                                       |
|                                                                              |
| "The B and D train batteries entered service in 1978.  The service date is   |
| determined from the time they were placed into the service for which they    |
| were intended.  For these batteries that is the date they were placed on     |
| float charge.  The batteries have a qualified life of 20 years.  Therefore,  |
| they reached the 17-year, 85% Technical Specification limit in 1995 per ESR  |
| 87-VT-065."                                                                  |

| "The following is the time line for battery testing in Unit 2:               |
| 1993, routine 60-month performance test (using OPSP06-DJ-0005) in accordance |
| with 4.8.2.1e taking credit                                                  |
| for the service test.                                                        |
| 1994, routine 18-month service test (using OPSP06-DJ-0004) in accordance     |
| with 4.8.2.1d.                                                               |
| 1995, special 18-month performance test (using OPSP06-DJ-0005) in accordance |
| with 4.8.2.1 f.  No service                                                  |
| test performed. Credit for the service test is taken based on the            |
| performance test in accordance with                                          |
| OPSP06-DJ-0005, Section 1.1.                                                 |
| 1997, special 18-month performance test (using OPSP06-DJ-0005) in accordance |
| with 4.8.2.1f.  No service                                                   |
| test performed.  Credit for the service test is taken based on the           |
| performance test in accordance with                                          |
| OPSP06-DJ-0005, Section 1.1.                                                 |
| 1998, (end of qualified life) the batteries were replaced."                  |
|                                                                              |
| "The testing performed does not meet the requirement of Technical            |
| Specification 4.8.2.1e in that Technical Specification 4.8.2.1d only allows  |
| credit for the service test once in a 60-month interval. Since the new       |
| 60-month interval started in 1993, the testing in 1995 and/or 1997 should    |
| have included both a service test and a performance test. Both service and   |
| performance tests are required in accordance with our commitment to Reg.     |
| Guide 1.129, Rev. 1."                                                        |

| The licensee declared the Technical Support Center ventilation system        |
| inoperable after a system test indicated high supply flow rates and low      |

| The licensee has declared the 'A' train of the Standby Gas Treatment System  |
| (SGTS) inoperable due to concerns regarding the seismic qualification of     |
| certain piping supports. These supports do not appear to meet seismic design |
| requirements. The 'B' train of SGTS is not affected by this issue.  The NRC  |
| resident inspector has been informed of this notification by the licensee.   |

| OFFSITE NOTIFICATIONS REGARDING FIRE IN OWNER CONTROLLED AREA                |
|                                                                              |
| The licensee notified the Maryland State Fire Marshall and Department of     |
| Natural Resources regarding a fire within the owner controlled area.  A fire |
| of approximately 30' x 15' was discovered in the Camp Canoi recreation area  |
| at a location adjacent to the site of a fire on 01/12/99. (Refer to event    |
| #35244.) The fire was extinguished as a result of efforts by both onsite and |
| offsite fire fighters. An eyewitness recorded the license number of a        |
| vehicle seen leaving the area, and the Maryland State Police was able to     |
| identify the suspect.                                                        |

| The NRC resident inspector has been informed of this event by the licensee.  |

| MOMENTARY LOSS OF PROMPT NOTIFICATION SYSTEM                                 |

| The licensee reported that both the primary and backup lines to the National |
| Weather Service (NWS) office in Jacksonville, FL, were lost for three        |
| 10-second periods during a 2-minute interval beginning at 1648 EST.  The     |
| licensee contacted the NWS office but was unable to determine the cause of   |
| the communications interruptions.  No further interruptions have been noted  |
| since 1650 EST.                                                              |
|                                                                              |
| The NRC resident inspector will be informed of this notification by the      |
| licensee.                                                                    |

| LOSS OF EMERGENCY NOTIFICATION SYSTEM (ENS) AND COMMERCIAL TELEPHONES        |

| At 0300, it was discovered that the FTS-2000 ENS telephone line and the      |
| offsite commercial telephone lines were out of service. The cause of this    |
| telephone outage is unknown at this time.  However, the licensee stated that |
| there does not seem to be a trunk line problem.  The licensee has contacted  |
| Ameritech and a local telephone representative. Offsite communications are   |
| available via the use of cellular telephones and base radios to the Michigan |
| State Police and to the licensee's power control (load dispatcher).  Outside |
| calls can be made from those locations if it is necessary to relay           |
| information.                                                                 |
