Reactor Operator/Senior Reactor Operator License Verification

Persons seeking to verify that an individual holds an active reactor operator or senior reactor operator license, or to obtain a copy of such license, may submit a Freedom of Information Act (FOIA) request for a copy of the license. Requesters seeking their own license must include identity verification documentation in accordance with 10 CFR 9.54. Requesters seeking the license of another individual must include identity verification and third-party authorization.  NRC Form 507 may be used for this purpose.
