- 10CFR PART 21 REPORT -

FOUR SAFETY-RELATED 480-VOLT WESTINGHOUSE MODEL #DB-50 CIRCUIT BREAKERS WERE SENT TO NUCLEAR LOGISTICS, INC, FT WORTH, TEXAS, AN APPROVED 10CFR50, APPENDIX B QUALIFIED REPAIR FACILITY, FOR OVERHAUL. THE BREAKERS WERE RETURNED TO UNIT 2 OUTSIDE THE MANUFACTURER'S TOLERANCES AND NOT MEETING THE REQUIREMENTS OF THE LICENSEE CLASS "A" (SAFETY- RELATED) PURCHASE ORDER.

THIS CONDITION WAS DISCOVERED DURING PRE-INSTALLATION TESTING BEFORE THE BREAKERS WERE INSTALLED IN UNIT 2. IF INSTALLED, THESE BREAKERS MIGHT HAVE MALFUNCTIONED WHEN CALLED UPON TO PERFORM THEIR SAFETY-RELATED FUNCTION.

THE LICENSEE PLANS TO REPAIR AND TEST THESE BREAKERS (AND THE REMAINING 46 SAFETY-RELATED 480-VOLT BREAKERS) ON-SITE PRIOR TO INSTALLATION.

THE LICENSEE PLANS TO SUBMIT A WRITTEN PART 21 REPORT TO THE NRC.

THE LICENSEE WILL INFORM THE NRC RESIDENT INSPECTOR.
