RE:            Potential 10CFR 21 Condition

SUBJECT:       Peerless-Winsmith D.C. Motors supplied as spare parts for
               Limitorque Valve Actuators

Please be advised that Limitorque Corporation has discovered, through
Material Verification, the seals which were supplied n some Peerless-
Winsmith D. C. motors were fabricated from a material other than VITON. 
Limitorque has completed an evaluation of the Peerless-Winsmith D. C. 
motors which were not supplied with VITON seals and has concluded the
seal material is BUNA-N.  The BUNA-N seal is rated for high temperature 
range of 210 to 250 degrees F, whereas the VITON seal is rated for a high
temperature range of 450 to 600 degrees F.  This condition could
potentially be reportable depending on the operating parameters of the
licensee.  Therefore, pursuant to the requirement of 10CFR Part 21 herein
provided is notification of a potential defect in Peerless-Winsmith D.C. 
motors supplied as spare parts for Limitorque valve actuators.

Identification of Basic Component

The affected component is the Peerless-Winsmith D. C. motor that provides
input power to actuator types SMB/SB/SBD 000 through 5 and SMB 000
through 5/HBC 0 through 7 combinations.

Discovery Date

Limitorque was notified of the failed seal on 02-22-95 and completed
their evaluation of affected equipment on March 6, 1995,

Nature of the Defect

The affected VITON seal of the Peerless-Winsmith D.C. motor prevents
grease from entering the motor when assembled to a Limitorque valve
actuator.  The BUNA-N seal being rated for a high temperature of 250
degrees F could fail when subjected to a transient temperature of 340
degrees F and thus allow grease to ingress into the motor.  This defect
could result in failure of the motor.

Affected Equipment

The equipment affected is (14) Peerless-Winsmith D. C. motors furnished
with BUNA-N seals.  The affected motor serial numbers are shown on
ATTACHMENT 'A'.

Limitorque Corporation Corrective Action

The incorrect seal material was discovered during Material Verification
of Motor Kits supplied with Peerless-Winsmith D. C. motors.  Limitorque
Corporation contacted Peerless-Winsmith and discovered that Peerless-
Winsmith had made an unauthorized change in the material for this seal.

In order to prevent recurrence of this defect, Limitorque Corporation
will request Cause and Corrective Action from Peerless-Winsmith to
prevent recurrence.  Additionally, Limitorque will increase our Motor Kit
Inspection Plan to require visual inspection of components upon receipt
and a Motor Kit from each supplied frame-size motor be tested on a
monthly basis.

The affected Licensees/Customers will be contacted by a separate letter
from Limitorque Corporation.

Licensee/Customer Corrective Action

Ensure operability justification of the Peerless-Winsmith motor with a
BUNA-N seal or contact Mr. Roland Odrick for a Returned Goods permit to
replace the seals at no charge (804) 528-4400 Ext. 9731 or Fax (804) 845-
9736.

- LIMITORQUE VALVE ACTUATOR DC MOTORS CONTAIN WRONG TYPE OF
GREASE SEAL -

ON 03/06/95, LIMITORQUE DISCOVERED THAT CERTAIN PEERLESS-WINSMITH DC
MOTORS SUPPLIED AS SPARE PARTS FOR CERTAIN LIMITORQUE VALVE
ACTUATORS
CONTAIN THE WRONG TYPE OF GREASE SEAL.  THIS SEAL PREVENTS GREASE
FROM
ENTERING THE MOTOR WHEN ASSEMBLED TO A LIMITORQUE VALVE
ACTUATOR.  THE
SUBJECT MOTORS WERE SHIPPED TO LIMITORQUE WITH AN IMPROPER BUNA-N
SEAL
(RATED FOR A HIGH TEMPERATURE RANGE OF 210 TO 250 DEGREES F) INSTEAD
OF A
PROPER VITON SEAL (RATED FOR A HIGH TEMPERATURE RANGE OF 450 TO 600
DEGREES F).  THE IMPROPER BUNA-N SEAL COULD FAIL WHEN SUBJECTED TO A
TRANSIENT TEMPERATURE OF 340 DEGREES F AND THUS ALLOW GREASE TO
INGRESS
INTO THE MOTOR CAUSING THE MOTOR TO FAIL.

THESE MOTORS PROVIDE INPUT POWER TO LIMITORQUE VALVE ACTUATOR
TYPES
SMB/SB/SBD 000 THROUGH 5 AND SMB 000 THROUGH 5/HBC 0 THROUGH 7
COMBINATIONS.
