Discussion:
As documented in Morning Report 2-2005-0014, the licensee defueled the reactor and performed ultrasonic examination of 100% of the baffle-to-former bolts. No baffle-to-former bolt failures were identified. The licensee is continuing to evaluate small gaps noted in several baffle plate corners and the cause of baffle-to-fuel grid strap interactions.
The licensee will develop an engineering evaluation to support restart of the unit, including an evaluation of the structural integrity of the baffle plates, the potential for loose parts, and the consequences of continued baffle-to-fuel grid strap interactions. As a compensatory measure to address the increased risk of fuel leaks resulting from baffle-to-fuel grid strap interactions, the licensee will continue to maintain more restrictive administrative limits on their fuel reliability index. Region II, with support from the NRR staff, will review the licensee's engineering evaluation.
