During defueled conditions, the Residual Heat Removal (RHR) pump A was   
started to fill and vent the RHR system. The pump tripped moments later. 
Investigation determined that a back casing ring capscrew had become     
loose and subsequently lodged between the impeller and the back casing   
ring. RHR pump B was inspected and found to have a similar condition. In 
addition, the spare back casing rings in the warehouse were inspected and
found to also have inadequately staked capscrews.                        
                                                                         
Root cause of this event was determined to be a failure of the           
manufacturer to properly stake the back casing ring capscrews. Each      
capscrew is torqued and staking is performed as a redundant measure to   
hold the capscrews in place. The improperly staked capscrews lost torque 
and subsequently backed out. This could potentially cause both trains of 
ECCS equipment to be rendered inoperable.

Information Notice 2002-35, "CHANGES TO 10 CFR PARTS 71 AND 72 QUALITY   
ASSURANCE PROGRAMS," was issued on December 20, 2002.                    
                                                                         
This notice was issued to all holders of 10 CFR Part 71 quality assurance
(QA) program approvals and all 10 CFR Part 72 licensees and certificate  
holders to remind them of the requirement to obtain NRC approval of all  
changes to Parts 71 and 72 QA programs before implementation of the
