Subject: LOW LEVEL RADIOACTIVE WASTE FORM PROBLEM IDENTIFIED

Discussion:

On April 9, 2001, Connecticut Yankee was informed that a High Integrity
Container shipped from the Haddam Neck site was tested upon arrival at
the Barnwell, South Carolina, Low-Level Radioactive Waste Disposal
facility and found to contain free standing water in excess of burial
site limits.   The liner was punched and 4.41 gallons of water was
drained.   The allowable amount of free standing water for this waste
volume was 2.78 gallons, i.e., 1% of the waste volume.  The liner
contained wet filters removed from the reactor vessel cavity and small
bags of dry active waste (DAW).   Although the waste had been loaded and
dewatered in accordance with the Haddam Neck Process Control Program and
approved procedures, small amounts of water had not drained completely
from the voids around the bags of DAW. The liner was ultimately accepted
by the disposal site contractor and buried that afternoon.  The licensee
initiated a corrective action report and on April 12, notified the Region
I office as a courtesy.

Regional Action:

An NRC inspector was at the Haddam Neck site the week of April 16, 2001,
and reviewed with cognizant licensee representatives the cause and
corrective actions for this occurrence.  In addition, the NRC inspector
informed the licensee of the applicability.  10 CFR Part 61.56 waste
characteristics requirements.  Enforcement action will be taken as
warranted.

Subject: UNUSUAL EVENT DECLARED DUE TO FIRE IN UNIT 1 CONTAINMENT BLDG.
Reportable Event Number: 37941

Discussion:

At 2:06 a.m. CDT on April 24, 2001, the licensee declared an Unusual
Event under its emergency plan after discovering a fire in a bag of rags
in the Unit 1 containment building. The fire was located on the manway
platform of the "A" steam generator. The plant fire brigade responded and
extinguished the fire approximately 20 minutes after the fire was
reported to the control room. The cause of the fire is unknown at this
time and the licensee has quarantined the platform on which the fire
occurred as part of its investigation. Unit 1 is currently in a refueling
outage with no fuel in the reactor. The licensee terminated the Unusual
Event at 2:43 a.m. CDT. The licensee was planning to make a press release
but there has been no media interest at this time. The State of
Wisconsin, the County of Manitowoc, and the County of Kewaunee have been
notified.

Regional Action:

The resident inspectors responded to the plant and inspected the fire
scene. No significant problems were identified immediately but potential
problems from the water and fire extinguisher dry chemical used to
extinguish the fire, and from the smoke generated during the fire have
not yet been evaluated.
