Subject: SERIOUS PERSONNEL INJURY DURING REFUELING OUTAGE
Reportable Event Number: 35616

Discussion:

While lowering an approximately 2000 pound flow diffuser from the high
pressure turbine shell during the main turbine retrofit work, a worker
lost a substantial portion of one finger when the cribbing and jack used
to support the diffuser failed and pinned the worker's hand between the
diffuser and an I-beam used as part of the cribbing assembly.

Because of the extent of the injury, the worker was evacuated for offsite
medical attention prior to site personnel completing a contamination
survey.  At the hospital, the individual was determined not to be
contaminated.

The licensee stopped work on the turbine retrofit work and conducted a
safety standdown for all personnel.

Regional Action:

The resident staff continues to follow the licensee's actions in response
to this event.  NRC staff informed the OSHA area office in accordance
with the NRC/OSHA memorandum of understanding.

Subject: Point Beach Nuclear Plant Unit 1 Shut Down For Maintenance
         Outage


Discussion:

On April 23, 1999, the licensee began a mid-cycle maintenance outage for
Unit 1. Major work scheduled for the planned four-day outage included:
repair of a control rod drive mechanism cooling fan, main generator
motor-operated disconnect repair, secondary system valve repairs, and
condenser waterbox cleaning. The outage is being conducted to bolster
reliability during the peak-demand summer months.

Regional Action:

The resident inspectors observed the reactor shutdown and will review
selected maintenance activities during the outage.
