Subject: Recent Nuclear Reactor Operational Events that Have Occurred
         as a Result of Human Error

The NRC plans to issue an information notice describing recent
operational events at commercial reactors that involved human error. The
event discussions may include the Vogtle Unit 2 reactor trip of March 2,
1999; the Salem Unit 2 reactor trip of February 28, 1999; the Diablo
Canyon Unit 1 loss of spent fuel pool cooling on February 26, 1999; the
San Onofre Unit 2 loss of spent fuel pool cooling on February 1, 1999;
and the Ginna inadvertent isolation of the containment ventilation system
that occurred on February 27, 1999.
