EN Revision Text: SECONDARY CONTAINMENT INOPERABLE

The following information was provided by the licensee via phone and email:

"At 1640 EST, on December 4, 2024, secondary containment was declared inoperable. This condition is being reported as an eight-hour, non-emergency notification per 10 CFR 50.72(b)(3)(v)(C), as an event or condition that could have prevented fulfillment of a safety function.

"Secondary containment was declared inoperable due to a small hole discovered in the service water piping within secondary containment. The effective safety function was restored at 1730 EST by installation of a temporary repair and secondary containment has been restored to operable.

"There was no impact on the health and safety of the public or plant personnel.

"The NRC Resident Inspector has been notified."

The following information was provided by the licensee via phone and email:

"The purpose of this notification is to retract EN 57454 which was made on December 4, 2024, at 1901 EST.
"At 1640 on December 4, 2024, secondary containment was declared inoperable due to a small hole discovered in the service water piping within secondary containment. This condition was reported as a non-emergency notification per 10 CFR 50.72(b)(3)(v)(C).
"Subsequent to this, additional analysis of the hole size determined that secondary containment was operable. Secondary containment maintained adequate leak tightness thereby ensuring the technical specification required vacuum could be established and maintained, and releases to the environment following a postulated accident would be within regulatory limits.
"The operability determination for secondary containment has been updated indicating that secondary containment operability was never lost for this event. As a result, there was not a condition that could have prevented the system from fulfilling the safety function. Secondary containment was operable during this time.
"The NRC Resident Inspector has been notified."

AGREEMENT STATE REPORT - MISSING DEPLETED URANIUM SOURCE

The following information was provided by the Florida Bureau of Radiation Control (BRC) via email:

"BRC received notification of a missing 270-pound depleted uranium source on January 14, 2025. Baptist Hospital discovered the missing source during an inventory performed in October, 2024. The last documented inventory where this material was observed occurred in 1998.

"Estimated activity: 40mCi"

THIS MATERIAL EVENT CONTAINS A 'Less than Cat 3' LEVEL OF RADIOACTIVE MATERIAL

Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks. AGREEMENT STATE REPORT - UNPLANNED CONTAMINATION DUE TO FIRE

The following information was provided by the Tennessee Division of Radiological Health via email:

"An unplanned fire occurred during the cutting of waste material (steam generators) in a restricted area. Memphis Fire Department used light water and foam to contain the fire. The material involved (plutonium) did exceed five times the annual limit on intake. Berms were established to contain any contaminated water. The building did have negative ventilation. Particulate filters showed background levels and air sampling showed no contamination outside. Also, there was no damage to the facility or injuries to personnel.

"Corrective actions or reports as well as additional information will be updated with a NMED report within 30 days."

AGREEMENT STATE REPORT - LOST SOURCE

The following information was provided by the Illinois Emergency Management Agency (the Agency) via phone and email:

"On January 8, 2025, the Agency was notified by representatives of the Lincoln Memorial Hospital of two sources that were missing after being offered for shipment through a common carrier. The two sealed Co-57 calibration sources collectively accounted for approximately 0.5 millicuries. While the sources were offered for shipment on October 23, 2024, the fact the sources never reached their destination was not discovered until an audit on January 8, 2025. After having received no further information from the carrier, the package is being declared as lost. The Agency staff will evaluate the timeliness in reporting, the level of accountability exerted over the sources, and root cause for the loss in a reactive inspection this coming week. Updates will be provided as they become available."

60-DAY NOTIFICATION OF INVALID ACTUATION OF EMERGENCY DIESEL GENERATOR

The following information was provided by the licensee via phone and email:

"This 60-day optional telephone notification is being made in lieu of a licensee event report submittal as allowed by 10 CFR 50.73(a)(1). This notification is made pursuant to the reporting requirements specified in 10 CFR 50.73(a)(2)(iv)(A) for an invalid actuation of one of the systems listed in 10 CFR 50.73(a)(2)(iv)(B).
"At approximately 1705 EST on December 5, 2024, an invalid actuation of emergency diesel generator (EDG) '2' occurred. It was determined that the run timing relay failed causing energization of the starting air solenoid valves ultimately resulting in EDG '2' spuriously starting and running with no demand and no auto-start signals.
"The actuation was not initiated in response to actual plant conditions, it was not an intentional manual initiation, and there were no parameters satisfying the requirements for initiation of the system. Therefore, this event has been determined to be an invalid actuation.
"During this event, the actuation of EDG '2' was complete. Initiation of attendant instrumentation/controls and supporting auxiliary equipment was as expected for the actuation being outside normal EDG start logic. Emergency bus 'E2' remained energized from offsite power and EDG '2' did not tie to bus 'E2'.
"This event did not result in any adverse impact to the health and safety of the public."

The NRC Resident Inspector has been notified.
