AGREEMENT STATE REPORT - LOST STATIC ELIMINATOR

The following is a synopsis of information provided by the Colorado Department of Public Health and Environment via email:

The licensee reported the loss of one static eliminator (NRD model P-2021) containing 10 mCi of Po-210. This is being reported in accordance with Colorado Regulations Section 4.51.1.1 (10 CFR 20.2202(a)(1)).

THIS MATERIAL EVENT CONTAINS A 'Less than Cat 3' LEVEL OF RADIOACTIVE MATERIAL

Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks. CONTROL ROOM ENVELOPE INOPERABLE

The following information was provided by the licensee via email:

"At 0924 [EDT] on July 25, 2023, it was discovered that both trains of control room air conditioning system were simultaneously inoperable due to an inoperable control room envelope boundary. The boundary was restored at 0925 [EDT] on July 25, 2023. This condition is being reported as an eight-hour, non-emergency notification per 10 CFR 50.72(b)(3)(v)(D).

"There was no impact on the health and safety of the public or plant personnel.

"The NRC Resident Inspector has been notified.

"There has been no impact to Unit 3 which remains at 100% power."

PART 21 REPORT - MOTOR DRIVEN RELAY FAILED TESTING

The following information is a synopsis provided by the licensee via email:

River Bend Station completed an internal Part 21 evaluation concerning a motor driven relay that failed pre-installation testing due to a buildup of corrosion between the rotor and relay core. The relay was planned for use in the Remote Shutdown System. The NRC Resident has been notified. A written notification will be provided within 30 days.

Affected known plants include only River Bend at the time of the notification.
