AGREEMENT STATE REPORT - STUCK GAUGE SHUTTER
The following was received by email from the Texas Department of State Health Services:
"On March 1, 2021, the [Texas Department of State Health Services] was contacted by a licensee stating that during routine testing, the shutter on a nuclear gauge was found stuck in the open position. The gauge is an Ohmart Vega model SH-F2-30, containing a 100 milliCurie (original activity) cesium-137 source. Source serial number is 1965CG and the manufacturer is Ohmart. The licensee reported there is no risk of radiation exposure to members of the general public or workers at the facility due to the failure. The licensee stated they were working on a plan to repair the gauge and would provide that information once the plan is completed. Additional information will be provided as it is received in accordance with SA-300."

NOTIFICATION OF DEGRADED CONDITION
"At time 0323 [EST] on March 04, 2021, it was determined that the Reactor Coolant System (RCS) pressure boundary did not meet the acceptance criteria under ASME, Section XI IWB-3600, "Analytical Evaluation of Flaws." This condition will be resolved prior to plant start up. This event is being reported as an eight hour non-emergency notification per 10 CFR 50.72(b)(3)(ii)(A).
"There was no impact on the health and safety of the public or plant personnel. The NRC Resident has been notified."

INTERIM PART 21 REPORT - WELD PROCEDURES DO NOT MEET ASME CODE
The following is a summary of the report provided by the supplier:
On January 6, 2021, Crane Nuclear, Inc (CNI) received an e-mail message from a customer identifying that the weld procedures Crane summitted for use on the customer's purchase order were unacceptable, as the procedure did not meet the requirements of ASME Code Section IX. CNI immediately began research and determined the customer to be correct.
CNI is currently conducting research to determine where the affected weld procedures were used. The scope of supply is Code and safety related valve and valve parts supplied by CNI since 2009.
