| AT 1658 ON 03/22/99, THE LICENSEE NOTIFIED THE MARYLAND DEPARTMENT OF THE    |
| ENVIRONMENT THAT THE LICENSEE HAD SPILLED APPROXIMATELY FIVE GALLONS OF      |
| DIESEL FUEL OIL ONTO THE GROUND WHILE FILLING A FUEL TANK ADJACENT TO THE    |
| UNIT 2 MAIN TRANSFORMER OUTSIDE THE UNIT 2 CONTAINMENT BUTLER BUILDING.  THE |
| SPILL DID NOT REACH ANY STORM DRAINS OR THE BAY.  THE LICENSEE PLANS TO DIG  |
| UP THE OIL SOAKED SOIL AND DISPOSE OF IT PROPERLY.                           |

| AGREEMENT STATE REPORT (ALABAMA INCIDENT #99-6) - LOST I-125 MEDICAL THERAPY |
| SEEDS                                                                        |
|                                                                              |
| In the late afternoon of March 19, 1999, the State of Alabama was contacted  |
| by the University of Alabama at Birmingham (Alabama Radioactive Material     |
| License No. 266) concerning an incident involving lost or stolen Iodine-125  |
| medical therapy seeds.  The seeds, used for the treatment of prostate        |
| cancer, were delivered to the licensee's permittee (the medical user) by an  |
| employee of the licensee's Hazardous Material Facility on the afternoon of   |
| March 18, 1999.  Upon delivery, the individual placed the package on the top |
| of a trash can and then went to check on a patient.  Upon his return, the    |
| package was missing.  The package contained 107 seeds of I-125 with a total  |
| activity of 37.5 millicuries in a lead pig.  The licensee continues the      |
| process of interviewing housekeeping personnel and surveying trash dumpsters |
| to locate the package.  As of the close of business on March 22, 1999, the   |
| radioactive seeds had not been located despite extensive efforts by the      |
| licensee.                                                                    |

| AGREEMENT STATE EVENT (ALABAMA INCIDENT #99-5) - Damaged Troxler Moisture    |

| In the morning of March 18, 1999, the State of Alabama was contacted by      |
| William B. Bass, a radiological consultant on behalf of Bhate Engineering    |
| Corporation of Birmingham (Alabama Radioactive Material License No. 655)     |
| concerning an incident involving a damaged Troxler Moisture Density Gauge.   |
| Mr. Bass reported that at approximately 1500 CST on March 17, 1999, the      |
| radioactive device was accidentally driven over by a John Deere 770B-H Motor |
| Grader.  The Troxler gauge was in the closed shielded position when the      |
| accident occurred.  The impact bent the source rod and "crushed" the shield  |
| around the source.  Mr. Bass reported that he checked for contamination and  |
| finding none, secured the rod with duct tape to prevent any movement and     |
| placed the damaged device in the case and secured it in place with duct      |
| tape.  The area was surveyed and determined to be contamination free and a   |
| wipe test made along the seams of the damaged device.  Radiation exposure    |
| levels adjacent to the device were 2.5 mR/hr at six inches from the surface  |
| which is typical for an undamaged Troxler device.  The licensee intends to   |
| ship the damaged device to the manufacturer.                                 |

| INOPERABLE WHEN THEY FAILED TO START DURING A TEST.  THESE COMPRESSORS       |

| OPERATE THE HVAC SYSTEMS, AND START THE BACKUP GENERATOR.  ONE UNIT HAS A    |
| FAULTY COMPRESSOR AND THE OTHER HAS A FAULTY CIRCUIT BREAKER.                |

| THE LICENSEE PLANS TO SWAP CIRCUIT BREAKERS AND RESTORE ONE UNIT TO OPERABLE |
| STATUS.  THE LICENSEE DECLARED THE EOF INOPERABLE.                           |

| THIS CONDITION IS A VIOLATION OF THE FACILITY OPERATING LICENSE, ATTACHMENT  |
| I(e)(3).                                                                     |

| THE LICENSEE HAS NOTIFIED STATE AND LOCAL OFFICIALS, LICENSEE EMERGENCY      |
| RESPONSE PERSONNEL, AND THE NRC RESIDENT INSPECTOR THAT THE EOF IS           |
| INOPERABLE AND TO RESPOND TO THE BACKUP EOF LOCATED AT THE BAXTER WILSON     |
| POWER STATION IN VICKSBURG, MS, APPROXIMATELY 30 MILES FROM THE SITE, IF THE |
| EOF IS REQUIRED TO BE STAFFED.                                               |
