OFFSITE NOTIFICATION

The following information was provided by the licensee via phone and email:

"On November 3, 2026, water from the unit 3 sump was being pumped to the oil waste separator (OWS) at approximately 1015 [PST], a clamp on the hose coupling failed, causing the coupling to separate. Water was released into the environment along the northwest side of the MHF [Material Handling Facility] tent. Upon discovery of the leak, immediate actions were taken to stop the pumping operation.

"On November 4, 2026, at 0930 [PST], San Onofre Nuclear Generating Station (SONGS) notified the San Diego Regional Water Quality Control Board of unplanned discharge associated with the SONGS NPDES [National Pollutant Discharge Elimination System] permit. This report is submitted in accordance with 50.72(b)(2)(xi) for contact with an offsite agency. In addition, this report is part of the voluntary communication protocol described in NEI 07-07 for the groundwater protection initiative.

"Approximately 900 gallons were released, samples were taken and the results are as follows: pH 11.6; total suspended solids 68.4 ppm; Tritium 8.36E-6 microCi/ml (8,360 pCi/l); gamma not detected.

"Based on currently available information, [Southern California Edison] SCE believes there is no adverse impact to ground water and ocean water quality."

AGREEMENT STATE REPORT - MEDICAL EVENT

The following information was received from the Arkansas Department of Health (the Department) via email:

"On October 29, 2026, the Department received a notification regarding NEA Baptist Memorial Hospital.

"The event occurred on October 27, 2026, at 0830 CDT. A patient was given 27 mCi of Tc-99m Sestamibi intravenously for a parathyroid image. Two days later, on October 29, 2026, the provider that ordered the exam called to question the radiologist report. Upon further investigation, the written order was for a thyroid uptake scan. It was determined that the patient had received a misadministration under 10 CFR 35.3045, wrong radionuclide use. The Department is reporting the notification of this event to the NRC under SA-300 procedures. A written report was given to the Department on October 29, 2026. The licensee has initiated retraining of nuclear medicine staff regarding order verification and notes that training has been completed. The Department will note these future preventative measurements and verify documentation on the next inspection.

"No immediate adverse health effects are expected.

"Details on the cause of the event occurrence were not included in the written report provided to the Department and will be determined through further investigation."

AGREEMENT STATE REPORT - MEDICAL EVENT

The following information was provided by the Kentucky Division for Public Health and Safety, Radioactive Materials Section, via email:

"During a Y-90 microsphere (TheraSphere) procedure performed on October 30, 2026, an issue occurred that resulted in the patient receiving approximately 19 percent of the prescribed dose. No spill or contamination was identified. Because the administered dose differed from the prescribed dose by greater than 20 percent, this event meets the definition of a reportable medical event.

"The authorized user has reviewed the event and determined that the delivered dose is not sufficient for treatment of the affected lobe. The patient will be brought back for additional treatment. Any future treatment will be performed under a separate written directive. The authorized user communicated this to the patient."

A Medical Event may indicate potential problems in a medical facility's use of radioactive materials. It does not necessarily result in harm to the patient.

AGREEMENT STATE REPORT - SHUTTER MALFUNCTION

The following is a summary of information provided by the Illinois Emergency Management Agency (the Agency) via email:

On October 30, 2026, during a routine inspection, an Agency inspector found that the shutter for the 5.4 mCi Cs-137 source in a Troxler model 3440P moisture density gauge (S/N 62163) would not close completely. A survey of the gauge showed a small area of streaming on the exterior of the gauge transport container reading 20 mrem/h.

The Agency inspector determined that given the typical use of the gauge and the established licensee procedures for storage of the device during transport, it is unlikely that any individual received an exposure exceeding regulatory limits.

The licensee and inspector added shielding to the gauge transport container to compensate for the shutter gap and verified the transportation index on the container shipping label was still met. The gauge, in its transportation container, was placed in secure storage pending instruction from the manufacturer regarding proper shipment for repair.
