AGREEMENT STATE REPORT - CABLE DISCONNECTED FROM RADIOGRAPHY SOURCE
The State of Kentucky reported a source containing 39 Curies of Ir-192 disconnect from a QSA model 660A radiography camera during radiography of a boiler. There was no leakage or personnel exposure during this event. The licensee contacted a contractor for retrieval. The source was secured and placed inside the camera. The State will follow up with additional details.
