AGREEMENT STATE REPORT - TROXLER GAUGE DAMAGED BY CONSTRUCTION EQUIPMENT

The following information was received from the Commonwealth of Virginia via facsimile:

"The licensee's portable density gauge (Troxler Model 3430 S/N 038435) was damaged on April 1, 2010 when it was struck by a piece of earth moving equipment. The accident occurred at a commercial construction site in Roanoke County, Virginia. The gauge operator secured the area of the gauge and telephoned the licensee's Radiation Safety Officer who performed an onsite investigation and a radiation survey. The source was in the shielded position at the time of the accident and the survey confirmed that it remained so. There was no damage to the source rod or handle and only minor damage to the gauge trigger and casing. The gauge was returned to the licensee's storage facility. There were no injuries or radiation exposures as a result of the accident."

ARKANSAS AGREEMENT STATE REPORT - MISSING TRITIUM EXIT SIGNS

The following was received from the State via email:

"The Department [State] received notice on April 1, 2010 by telephone and a written report on April 30, 2010, from Shaw Environmental, Inc. of seven (7) missing Tritium (H-3) exit signs.

"The signs were purchased and installed in a military recruiting center, located on University Avenue in Little Rock, Arkansas. The recruiting center was located in a mall complex that had been demolished and removed for the construction of a new shopping center.

"The signs were installed sometime between March, 2004 and October, 2005. The signs are described as:

"Manufacturer: SRB Technologies
"Model: BetaLux-E
"Activity: 10 Curies each
"Serial numbers: 318671-318675 and 318887-318888 (7 signs total)"

THIS MATERIAL EVENT CONTAINS A "LESS THAN CAT 3" LEVEL OF RADIOACTIVE MATERIAL

Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks. This source is not amongst those sources or devices identified by the IAEA Code of Conduct for the Safety & Security of Radioactive Sources to be of concern from a radiological standpoint. Therefore is it being categorized as a less than Category 3 source
