AGREEMENT STATE REPORT OF LOST IRIDIUM-192 SOURCE
The Alabama State Radiation Control Office reported that the University of Alabama Medical Center was missing a 250-millicurie iridium-192 source. The source is used for cardiology purposes and was last seen on April 22, 2002. The hospital was conducting searches to try to locate the missing source.
