K-RAY 706P SOURCE HOLDER DEVELOPED CRACK
The licensee discovered a crack, about 2-3 inches in length, in the K-Ray 706P source holder, possibly in a weld. The gauge, containing a 10-mCi Cs-137 source, is in a radioactive labeled room under a coal pipe at the Pleasant Prairie Plant, and it has been wrapped with plastic and labeled with radioactive stickers. K-Ray has been notified and will replace the gauge before the end of the year. A 30-day written report will follow.

The licensee is retracting this report because the crack in the K-RAY 706P does not meet the definition of Substantial Safety Hazard found in 10 CFR 21.3. Per 10 CFR 21.3 Substantial Safety Hazard "means a loss of safety function to the extent that there is a major reduction in the degree of protection provided to public health and safety for any facility or activity licensed, other than for export, etc." The contact radiation level on the cracked K-RAY 706P was the same level as the K-RAY 706P without a crack, 1.5mrem/hr. Equipment model number was incorrect. The correct model number is 7062P instead of 706P.

OFFSITE NOTIFICATION REGARDING A BUNKER-C OIL SPILL AT THE FOSSIL UNITS
The following text is a portion of a facsimile received from the licensee:
"Units 1 and 2 (fossil) are notifying [the Department of Environmental Resource Management (DERM)] of [a] Bunker-C oil spill on [a] permeable surface. [It is] estimated that 100 - 150 gallons of bunker C [are] contained by an earth/rock berm around the fuel tank."
The licensee notified the NRC resident inspector.

AGREEMENT STATE REPORT - STOLEN MOISTURE DENSITY GAUGE
A Troxler nuclear gauge (Model 3430, Serial No. 26619) with the following sources: Cs-137, S/N: 75-8687, nominal activity 8 millicuries, and Am-241:Be, S/N: 47-23085, nominal activity 40 millicuries, was stolen from the Dallas, Texas, area during the hours of darkness December 15-16, 1999. A police report has been filed. The gauge was stolen from the back of a Licensee's pickup truck by cutting the security chain. The gauge has not been recovered.
A verbal report has also been filed with the NRC Regional Office. Delay in notification has been due to delays within this headquarters. Surrounding states and Mexico have also been notified of this theft.
