Gentlemen:
It has been discovered that the tubing used on the above contracts was
not properly certified by the Material Manufacturer. The tubing was
purchased and shipped from an approved vendor (Tube Sales). But, we
failed to notice that the CMTR did not contain a statement that the
tubing, manufactured by Sandvick Steel Co., was produced under their QSC
program. Sandvick is a Quality System Certificate holder, but this
material was not certified under their QSC program.
The tubing in question is 1" O.D. X 0.065 inch wall thickness, ASME SA213
TYPE 316. Tube stubs from this material were welded into our valve
bodies. A Liquid Dye Penetrant examination was performed on the weld
area, and a hydrostatic shell test and seat test was performed after
assembly.
The number of valves and their serial numbers are shown on the enclosed
Code Data Reports.
Since the tubing is certified to ASME SA213 TY 316 and the tests
mentioned above were performed, Dragon Valves. Inc. does not consider
this matter to be a substantial safety hazard.
The Owners (TVA) have been notified of the defect, concurrent to your
notification.
