of July 12, 1995, Revision to Instruction Manual 4235.
Rosemount Nuclear Instruments, Inc. (RNII) is writing to inform you of an
additional issue in an old revision to Instruction Manual 4235 for the
Model 1152 nuclear qualified transmitter. You were previously notified
in the July 12, 1995 letter that the October 1992 revision to Instruction
Manual 4235 inadvertently specified an incorrect process O-ring for use
with 1152 High Differential Pressure Transmitters (Model 1152HP). The
previous revision to Instruction Manual 4235, dated January 1988,
specified the correct O-ring for use with these transmitters. An older
revision, revision A, dated 11/82 also specifies the incorrect O-ring for
the 1152 High Differential Pressure Transmitters (Model 1152HP).
Model 1152 transmitters are offered in Gage, Absolute, Differential and
High Differential pressure types. Process O-rings are used to seal the
sensor module to the process flange interface. The material of the
process O-ring is ethylene propylene rubber for all types of Model 1152
transmitters. Model 1152HP transmitters are specified with process
O-rings having a higher durometer (hardness rating) than process O-rings
specified for Gage (except Range 9 and 0), Absolute, or Differential
types. The higher durometer O-ring provides sealing of the process
flange interface when subjected to operating pressures up to the
specified 4500 psig over the specified operating temperature range of the
1152 High Differential Pressure transmitter.
RNII has revised Instruction Manual 4235 to specify the correct process
O-ring kit part number and distributed them to users. This revision, MAN
4235A00 dated July 1995 should be used for all Model 1152 transmitters
(except Model 1152 T0280) regardless of the shipment date.

2.0 Identification of items supplied:
Instruction Manual 4235, Revision A (dated 11/82), for the Model
1152 transmitter incorrectly specified a lower durometer process O-
ring kit to be used with Model 1152 High Differential Pressure
transmitters (Model 1152HP). The Instruction Manual states on page
17, Table 3, "1152 Spare Parts", O-ring for Process Flange, Range
Code 3-8, Model 1152HP kit part number 01152-0067-0001. The correct
kit part number for this O-ring for Process Flange is 01152-0067-
0002.

4.0 Nature of the failure and potential safety hazard:
The difference between the process O-rings is the durometer
(hardness rating). The Model 1152HP, Range Code 3-8, transmitters
are specified to use a process O-ring having a higher durometer than
Model 1152 Gage (Except Range Code 9 and 0), Absolute, or
Differential types.
RNII does not have sufficient information to determine the safety
impact related to plant applications. Licensees must determine the
impact on plant operations and plant safety and take action as
deemed necessary.

5.0 The corrective action which is taken, the name of the individual or
organization responsible for that action and the length of time
taken to complete that action:
RNII has revised Manual 4235. The current manual, MAN 4235A00,
revision July 1995 was distributed to users October 13, 1995.
Mr. Van Sloun is responsible for any further action related to this
issue.
6.0 Any advice related to the potential failure of the item:
The end user must determine the full scope of this deviation as it
relates to plant operations and plant safety and take action as
deemed necessary.
