Part 21 2007-0001 - Potential Degradation of Hydraulic Snubber Fluid

On January 3, 2007, Florida Power and Light (FPL) Energy informed NRC that Duane Arnold nuclear power plant has identified foreign materials in the GE SF1154 snubber hydraulic fluid, which was supplied by Fronek Anchor Darling Enterprises, Inc. The fluid was manufactured by General Electric (GE) in June 2001 and shipped to Duane Arnold with GE batch number AD965.

The foreign materials in question had the appearance and texture of paraffin wax. A sample analysis of the fluid indicated that foreign material was silicone. It is suspected that silicone has been introduced during processing or repackaging, or from some form of reaction with the plastic container. The presence of foreign materials in hydraulic fluid could restrict fluid flow through the orifice in the snubber and cause the snubber to remain locked after activation and prevent normal system movement. FPL Energy has removed this fluid from service at Duane Arnold.

On January 8, 2007, the NRC staff contacted Fronek Anchor Darling Enterprises, Inc. and learned that the same defective fluid has been supplied to Pilgrim and Browns Ferry nuclear power plants. Since other utilities and GE customers might have also received the same defective snubber hydraulic fluid through other suppliers, the NRC staff has prepared this morning report to inform all licensees of this defect. The NRC will continue to obtain, evaluate, and publish additional information concerning this defect.

Dominion Energy Kewaunee, Inc., (DOM) announced recent changes in its upper management structure at Kewaunee: Kevin Davison, currently the Director of Operations and Maintenance at Kewaunee, has been replaced by Mike Crist, effective February 1st, 2007. Mr. Crist's prior assignment was the Operations Manager at Surry. Mr. Davison will become the Recovery Director at Kewaunee effective February 1st, 2007. The Recovery Director is a newly created position whose goals include improved plant performance and overall plant excellence.
