AGREEMENT STATE REPORT - MISSING DENSITY GAUGE

The following was received from the Colorado Department of Public Health and Environment (CDPHE) via email:

"[A Troxler 3440 (SN 23106) containing not more than 333 MBq (9 mCi) of cesium-137 and 1.63 GBq (44 mCi) of americium-241:beryllium; or 2.44 MBq (66 microCi) of californium-252] was reported missing after a licensee was unable to account for it during a routine inventory check. The material was noticed missing at approximately 1000 MDT on Wednesday, October 14, 2020. The event was reported to the CDPHE at approximately 1700 on October 14, 2020.

"No signs of a burglary at the facility are present. The last entry for the gauge in the utilization log is August 19, 2020. The licensee suspects that a former employee stole the gauge, the employee to last check-out the gauge was terminated on bad terms.

"The licensee is in the process of reporting the theft to the local police."

AGREEMENT STATE REPORT - LOST GAUGES

The following was received from the state of Tennessee via email:

"General licensee, Blues City Brewery, reported the loss of 3 gauges after contractor work that involved the movement of the gauges. Blues City reported the loss. It is possible that the gauges could have made it into the local scrap metal stream. From the scrap metal facility, the load that might have included the gauges went to a shredding facility in Alabama. The State of Alabama Radiation Control has been notified in case gauges are discovered at the shredding facility. The gauge is below:

The following was received from the state of Tennessee via email:

"Energy Solutions experienced a fire in the gas furnace 'Drum Drying Operation' of the Liquid Volume Reduction Facility (LVRF) building, which is a sub building off of the Incinerator building located at the 1560 Bear Creek Road, [Oak Ridge, TN] facility. The drums being processed contained Energy Solutions secondary waste with the following listed source term associated with the containers: C-14 (0.5 mCi), Co-60 (2.2 mCi), Cs-137 (0.96 mCi), Fe-55 (2 mCi), H-3 (4 Ci), and Tc-99 (98 mCi).

"Estimated 0.03 mrem dose to the public as a result of the event."

AGREEMENT STATE REPORT - LOST AND FOUND PORTABLE GAUGE

The following was received from the Utah Division of Radiation Control (the Division) via email:

"A portable gauge user failed to adequately secure the transportation case containing a Troxler 3430 gauge when leaving the jobsite. The user noticed the gauge was missing after travelling some distance from the jobsite. The user retraced his path back to the jobsite, but was unable to locate the gauge. The licensee then contacted the Division [at 1422 MDT] to report that the gauge was missing. During a subsequent telephone call [at 1503 MDT], the licensee informed the Division that the gauge had been located during an additional search by the user. The gauge was found just outside the jobsite entrance. The Troxler gauge contains a 40 mCi Am-241/Be sealed source and an 8 mCi Cs-137 sealed source."

AGREEMENT STATE REPORT - GREATER THAN INTENDED DOSE TO LIVER LOBE

The following was received via email from the California Radiation Control Program:

"On October 16, 2020, the [Radiation Safety Officer] RSO of Stanford University emailed [the Radiation Health Branch] RHB to inform a medical event with a Y-90 patient treatment. The physician mistakenly delivered the larger dose (approximately 30 mCi) to the liver lobe that was to get the smaller dose (approximately 13 mCi). The dose to the second lobe was adjusted with left over Y-90 from the dose draw to give the proper dose to the other lobe (approximately 30 mCi). So one lobe received much greater than the intended dose, while the other lobe received the proper dose. RHB will follow up on this investigation."

EN Revision Text: CONTROL ROOM ENVELOPE INOPERABLE

"On October 18, 2020 at 2313 CDT, Arkansas Nuclear One (ANO) discovered that 2VRA-1B (2VSF-9 outside air damper reserve bottle) was below 600 psig. This condition caused the control room envelope to be inoperable in accordance with OP-2104.007 Attachment L. ANO Unit 1 entered TS 3.7.9 Condition B for inoperable control room boundary. ANO Unit 2 entered TS 3.7.6.1 Action D for inoperable control room boundary. A procedurally controlled temporary modification was implemented to install a blank flange on the 2VSF-9 outside air damper. Both Units declared the control room boundary operable at 2358 CDT. The associated control room emergency recirculation fan remains inoperable with the blank flange installed. This is a 7-day shutdown-LCO for both units.

"The licensee informed the NRC Resident Inspector."

* * * RETRACTION FROM AARON TOSCH TO HOWIE CROUCH ON 10/24/2020 AT 1657 EDT * * *..

"Previously, Entergy notified the NRC that ANO control room envelope was inoperable due to 2VRA-1B (2VSF-9 outside air damper reserve bottle) was below required pressure of 600 psig. After additional engineering evaluation, it was determined the control room boundary remained intact for this condition.

"As documented in version 2 operability determination for condition report ANO-C-2020-2818, the control room ventilation boundary remained intact for the condition identified and was able to fulfill its function for the required 30-day mission time.

"In accordance with NUREG-1022, 'Event Report Guidelines 10 CFR 50.72 and 50.73,' a report may be retracted based on a revised operability determination. The CRE remained operable; therefore, this report may be retracted.

"The NRC Resident Inspector has been informed."
