Offshore Source Assembly Disconnect Incident

Discussion:

This is an update to Event Notification 41842 dated July 14, 2005, regarding a radiography source disconnect incident. The incident occurred while Global X-Ray & Testing Corporation (GXT) was conducting radiography under reciprocity on a pipeline laybarge in offshore Federal waters in the Gulf of Mexico. GXT is a state of Louisiana licensee.

Region IV has obtained additional information from the GXT Radiation Safety Officer (RSO). The RSO stated that the source assembly became disconnected from the drive cable while the source was positioned inside the collimator at the end of the guide tube. The workers immediately stopped work and continued to restrict access to the area. The RSO further indicated that, based on pocket dosimeter readings and that the worker's ratemeters did not alarm, he determined that the dose received by all personnel involved was well below regulatory limits. The licensee also subsequently sent the individuals' film badges off for immediate processing to confirm this determination.

The licensee plans to package and transport the damaged source assembly to AEA Technology Sentinel's facility in Baton Rouge, Louisiana, on Monday July 18, 2005, for initial examination. The source assembly will then be forwarded to AEA's source manufacturing facilities in Massachusetts for a failure analysis.

Additional Regional Action: Region IV personnel plan to meet with the GXT RSO and AEA staff at AEA's Baton Rouge facility on Monday, July 18, 2005, to discuss the incident and examine the damaged source assembly. This activity has been discussed with the State of Louisiana, Department of Environmental Quality.
